Applications For 2025 Are Now OPEN!!

You will need the following for your application entry:

Identification: The number on your Driver’s License, State or Country ID, Alien ID, or I-94 Card.

Residence in Tarrant County: The current address of your home or apartment. We are restricted to helping residents of Tarrant County by our bylaws.

Valid Email Address: We are sending notifications by EMAIL this year. There will be no letters in the mail. Your qualifying information will be sent to this address. Please enter it carefully!! You MUST have your OWN email address; our form restricts users to submitting ONE application. Gmail addresses are free.

Proof of Income for the Entire Family: This includes any of the following that apply to you and/or members of your family:  Your prior year’s Income Tax or IRS Tax Form 1040 OR paycheck stubs OR a letter from your employer stating your salary OR your current award letter(s) from TANF, SNAP food stamps, unemployment, child support, SSI/SSDI, and/or documentation of any other additional income/benefits you currently receive.   

Total family income must be less than 125% of the Federal Poverty Income Guidelines. 2025 Federal Poverty Guidelines Here!

Child’s Age: To be eligible for a gift card, your child must reside in the household and be between the ages of 4 and 16, be attending a Tarrant County school, and be born between June 1, 2009, and September 1, 2021.

Enrollment in School: You should enter your child’s unique school identification number. This may be found on a current report card or a dated letter from the school verifying that the child is enrolled. The category of Home-School is also acceptable.

The Small Print

You will have the opportunity to upload all of your documentation this year to our new application. If you choose this option someone will review your application and let you know when you are eligible to pick up your cards. Once you have this eligibility email, you can come to any location during any of our open hours to pick up your card(s).

Travis Avenue Baptist Church Hemphill @ Berry St. Fort Worth, TX 76110 for Fort Worth Applicants.

St. Vincent’s de Paul Catholic Church 5819 W. Pleasant Ridge Road, Arlington, TX 76016 for Arlington Applicants.
